Curious is the fact, that sometimes it helps if you scroll through the document before saving - but only sometimes and I am not able to find out in which cases... Increasing Tools→Options→Memory settings allowed me to open, change, save ARL_Workflow_User_Documentation.odt without losing images in 3.3.2 OOo and LibreOffice. I've just moved to LibreOffice which seems much better in many respects (can create .docx) but images still sometimes disappear (not always, which makes it all the more puzzling).
